• Post your Kaizens in any format.You can post your existing Kaizens available with you in any format (ie; Document file, EXL file, PPT file, PDF file) by clicking on this Post your kaizen link.
• You can post your kaizen on line by Clicking Submit Kaizen button.
• As well as off line by downloading pdf form locally by clicking on Download pdf link.
• Download pdf file and save on your computer.
• Read all instruction mentioned in PDF carefully.
• All * marked field are mandatory. You must fill all those field.
• After filling all details about your kaizen, click on save and continue button. You must enter your company name, Email address, and contact information and click on Submit button. Your Kaizen submitted and you will get reference id from web master for future reference.
• For submitting kaizen on line or off line user must follow all mentioned instruction.

5	Dept.	Mention the name of the department in which you are doing the Kaizen.	Essential
6	Date	State the date on which the Kaizen has been identified.
7	Problem	State & define the problem.i.e. what is the status before the Kaizen. This can be in the form of a Chart , Flow Process chart or a wrtie up etc.
8	Analysis	Write Why -- Why Analysis .The analysis should commence from the Problem stated earlier.In case of Idea Kaizen the why-why analysis is not required.
9	Root Cause	Mention the exact root cause of the problem. Avoid stating Design problem , Opertor problem etc.
10	Idea	It shall be opposite to Root Cause.
11	Counter Measure	What action you are taking in order to implement the Kaizen. It shall be objective not subjective.
12	Before Countermeasure	Photo / Process Flow chart / situation before counter measure.	Essential
13	After Countermeasure	Photo / Process Flow Chart / situation after counter measure is taken. In case of photo , the same shall be from same angle as that of ' Before Countermeasure'. Presentation style in 'Before Countermeasure' & 'After Countermeasure' shall be same; i.e. if a photo has been shown in ' Before' situation then a photo has to be shown in 'After' situation also. Please, do not copy the photo directly on the kaizen sheet. It should be either inserted or by converting it into JPEG file.	Essential
14	Implemented By	Name of the person who has imlemented the Kaizen.
15	Benefits / Results after implementation	Benefits achieved by implemention of this Kaizen in terms of Productivity / Quality / Cost / Delivery / Safety or Morale. In case of P / Q / C / D - Mention it specifically.
16	Scope & Plan for Horizontal Deployment	Mention any other area where this Idea Kaizen can be implemented.It may not be applicable in a countermeasure Kaizen.
17	Result Area	Tick the area in which you have been benifitted.	Essential
18	Kaizen No. - To be alloted by CII	Kaizen number will be alloted by CII representative after screening of the kaizen.
19	Sender's Details	Company Name ,Location , Sender Name , Sender contact details like Phone no. , E-mail id.	Essential

Note: Idea Kaizen must fill all essential fields mentioned in above sheet .

• Posted Kaizen will approved by screening committee.
• Screeners will send approval message to user.
• After approval your submitted Kaizen will gets displayed on web site.

SOP for Screening of Kaizen Sheet

• Following prerequisites must be checked in the Kaizen sheet for acceptance :
  • Industry Sector : To which sector the company belongs to (i.e Auto Components, Chemical, Electrical, Cement etc.). It should be clearly mentioned.
  • Kaizen Theme
  • Department : In which department the submitted Kaizen has been deployed (i.e. Production, Maintenance, Purchase/Office; etc).
  • Before Countermeasure: Pictorial, text or both explanation is a must.
  • After Countermeasure : Pictorial, text or both explanation is a must.
  • Result Area :
• It should be checked whether the Idea given in the submitted Kaizen sheet is understandable and is opposite to the root cause found by Why-Why Analysis.
• If required add on additional boxes like Idea, root cause.
• Screener can change the language if he/she finds it not understandable. The language should be easily understandable and grammatically correct.
• Once the Kaizen is screened by the screener, then it will be cleared by Head � Manufacturing Services, CII, before putting on the website.
• If the Kaizen is rejected by the screener, he/she should post his/her views/reason to the person who has submitted the kaizen with giving him ideas for improvement.
• Screening committee of five screener will be formed to be reviewed on yearly basis.
• The Kaizen received will be automatically be sent by webmaster on screener�s ID for the screener to screen the Kaizen. The webmaster will post the incoming Kaizens in rotation to all screeners.
